Description:

The RN who is responsible for the overall supervision of nursing care in the subacute unit in the long-term facility during their shift. Responsible and accountable for the provision of direct, age-specific, resident care to those assigned to their care for each established shift.

Subacute RNs:

The RN provides a subacute level of care for ventilator and trach-dependent residents according to the nursing processes of the facility and is responsible for coordinating the team approach to meet each resident’s individual needs. The RN subacute nurse provides clinical leadership for the other nursing staff and performs related duties as required. The RN supervises the LVNs, C.N.A. 's, and other ancillary nursing personnel involved in the delivery of resident care.

RN Supervisors:

The RN supervises the LVNs, C.N.A.’s, and other ancillary nursing personnel involved in the delivery of resident care. Responsible for ensuring all IV medications are also passed.

Essential Duties and Responsibilities:

  • Effectively communicate with medical professionals

  • Collaborate with physicians to create a Plan of Care for each patient

  • Consider the age of the patient and their emotional stability when determining appropriate treatment

  • Create and contribute to patient's medical records as needed

  • Respond to inquiries, concerns, and complaints from patients and their parents or guardians

  • Track and maintain medical supply inventory along with other staff members

  • Counsel patients and family members before, during, and after treatment

 

Education and Experience:

  • Graduate of an accredited school of nursing

  • Current unencumbered California RN licensure

  • Current CPR certification and subacute certification

  • Valid driver's license with an automobile that is in good working order and insured by state

  • Excellent observation, verbal and written communication, problem solving, and basic math skills

  • Visual acuity and hearing to perform required nursing skills

  • Experience in SNF

  • Proof of COVID-19 Vaccination.

 

Preferred Qualifications:

  • Subacute RNs: Nursing experience in subacute care with ventilator and trach care and knowledge of electronic medical record documentation

  • Shift RN Supervisor: Working knowledge with computer applications for electronic medical records, and knowledge of long-term care.
